In accordance with Texas House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, the following information is provided for clients and consumers.
Requesting Your Health Care Records
You have the right to request access to your health care records.

To request your records:

Requests may be made by email or through the secure client portal

Requests will be processed in accordance with applicable state and federal laws regarding access, timelines, and any allowable fees

Filing a Consumer Complaint
